Restart semaglutide after break: what the label says, by how long you stopped

By the NexLife Medical Team · Medically reviewed by Adam Kennah, MD · Updated

Restarting semaglutide after 2 or more missed weekly doses means a lower dose, back on the steps, per the Wegovy label (read November 1, 2026). At NexLife, a licensed provider sets your restart dose, and every dose costs $119 a month on the 12-month plan, or $139 month to month.

Missed 1 dose, 2 weeks or a month of semaglutide? What the label says

Count the weekly doses you’ve missed. Miss one, and the Wegovy label (revised June 2026) looks at your next dose: more than 2 days away, take it; less, skip it. Miss 2 or more in a row, and it restarts the dose steps lower, without naming the dose. Your provider picks it, so call before your next shot.

From the Wegovy prescribing information and Medication Guide (revised June 2026). Compounded semaglutide follows your prescription: if it says something different, your provider’s instructions come first.

Key takeaways

  • Missed 2 or more weekly doses in a row? The Wegovy label restarts its dose steps at a lower dose.
  • It names no restart dose: your provider picks it, and how fast you step back up.
  • Missed just one dose? Next dose more than 2 days away: take it now. Less: skip it, keep your usual day.
  • Check your units: from a 2.5 mg/mL vial, 1.7 mg is 68 units, but 0.5 mg is 20.
  • Opened a vial before the break? The FDA says to discard a multi-dose vial within 28 days after first use.
  • At NexLife, a lower restart dose costs the same: $119 a month on the 12-month plan, or $139 month to month.

The 2-day rule

What if you miss a semaglutide dose?

Look ahead, not back. If your next scheduled dose is more than 2 days (48 hours) away, take the missed one as soon as possible. If it’s less than 2 days away, skip it and keep your usual day. That’s the Wegovy label’s rule (revised June 2026). Unsure which applies? Ask your provider before you take it.

  1. Your dose day is Monday

You remember on Thursday, 3 days late. Monday is 4 days away, more than 2, so take it now. Then take your next dose on Monday as usual.

  2. You remember on Sunday

That’s 6 days late, and Monday is 1 day away. Skip the missed dose and take Monday’s dose as usual.

  3. You want a new dose day

The label’s patient guide allows it, as long as your last dose was 2 or more days before.

Seen a 5-day rule? That’s the Ozempic label, semaglutide for type 2 diabetes: take a missed dose within 5 days, or skip it once more than 5 days have passed. On a weekly schedule, both rules cut off about 5 days after the dose you missed.

On the daily Wegovy pill, it’s simpler: skip a missed tablet and take the next one the following day.

On compounded semaglutide, your prescription sets your schedule. If it says something different from this, your provider’s instructions come first. Unsure? Call or text us 24/7 at (949) 818-8000.

On tirzepatide instead? Its label’s rule is different: restarting tirzepatide after a break.

Restarting lower

Why restart semaglutide at a lower dose after a break?

To spare your stomach. The Wegovy label (revised June 2026) gives that reason itself: stomach side effects came most often while the dose was going up. So after 2 or more missed weeks, it restarts the steps at a lower dose and climbs again. How fast you climb back is your provider’s call.

  • Stomach side effects come mostly while the dose is going up. In Wegovy’s weight-loss trials, 73% of adults on Wegovy had them, against 47% on placebo. The label says they were reported most often during dose escalation.
  • The most common were nausea (44% vs. 16% on placebo), diarrhea (30% vs. 16%) and vomiting (25% vs. 6%).
  • The label’s slow steps lower that risk. It starts at 0.25 mg and steps up every 4 weeks, through 0.5, 1 and 1.7 mg, to 2.4 mg, the usual maintenance dose. If a step isn’t tolerated, it says to consider waiting 4 weeks before the next one.
  • After 2 or more missed weeks, a restart repeats some of those steps. The label says to restart them at a lower dose, without naming which one.
  • Fluids matter. The label warns that vomiting or diarrhea can lead to dehydration and kidney injury. It asks providers to check kidney function in people with these side effects, especially when the dose is started or raised.

So your provider looks at your whole break: how long you stopped, your last dose, how you felt on it and why you stopped. Then they set your restart dose and how fast you step back up.

Side effect figures from the Wegovy prescribing information (revised June 2026). These trials studied FDA-approved Wegovy; compounded semaglutide wasn’t studied.

Cost

What does restarting semaglutide cost?

For Wegovy’s self-pay pen, about the same as before: Novo Nordisk lists it at $349 a month from 0.25 mg to 2.4 mg, and $399 at 7.2 mg (checked November 1, 2026). Compounded programs priced by dose can cost less at a low restart dose. A flat price stays the same at every dose.

New to NovoCare Pharmacy? People new to it or to the Wegovy Savings Offer pay $199 for each of their first two fills, at 0.25 or 0.5 mg, through December 31, 2026. Every dose, priced: semaglutide price by dose.

At NexLife, your dose never changes the price. Compounded semaglutide is $119 a month on the 12-month plan, $125 on 6 months, $132 on 3 months, or $139 month to month, at every dose. Restarting at 0.25 mg costs the same as your old dose, and so does each step back up. There’s no membership fee. Compounded semaglutide isn’t FDA-approved; Wegovy is.

Switching from another licensed provider with an active prescription? If you pay less there for compounded semaglutide, our Price Match Guarantee† matches your regular monthly price on our month-to-month plan. See the terms.

Lost your coverage? Here are six options, priced. On Medicare Part D, the GLP-1 Bridge lets people who qualify pay $50 a month for Wegovy through December 31, 2027, with prior authorization: who qualifies.

Units

How many units is your restart dose?

Fewer than before, if your dose is lower. Units on a U-100 syringe equal the dose in mg divided by your vial’s strength in mg/mL, times 100. So the same restart dose can be a different number of units in a new vial. Your prescription sets the dose; this math is only for checking it.

Units of compounded semaglutide on a U-100 insulin syringe (100 units = 1 mL): units = dose in mg ÷ vial strength in mg/mL × 100. For checking your math, not choosing a dose: your prescription sets it.

An example. On 1.7 mg from a 2.5 mg/mL vial, you drew 68 units. If your provider restarts you at 0.5 mg, it’s 20 units from the same vial.

Check the strength on every new vial. 10 units is 0.25 mg from a 2.5 mg/mL vial, but 0.5 mg from a 5 mg/mL vial. The FDA has reports of side effects that may be linked to dosing errors with compounded semaglutide.

Restarting from a vial you opened before the break? Check the date first. The FDA says to discard a multi-dose vial within 28 days after first use, even if some is left. More in compounded semaglutide storage.

Other doses and strengths are in our semaglutide dosage chart. Unsure about a number? Call or text us 24/7 at (949) 818-8000 before you inject.

In their words

What does the FDA say to check before you restart semaglutide?

Three FDA lines for your first dose back: how old your vial is, its strength, and how to measure your dose.

Restarting from a vial you opened before the break? Check its first-use date against the FDA’s advice.

“Discard the vial within 28 days after the first use, even if medication remains in the vial.”

U.S. Food and Drug Administration, FDA’s Concerns with Unapproved GLP-1 Drugs Used for Weight Loss

This is why the same restart dose can be a different number of units in a new vial: check the strength on every label.

“Product concentrations also may vary depending on the compounder, and a single compounder may offer multiple concentrations of compounded semaglutide products.”

U.S. Food and Drug Administration, FDA alerts health care providers, compounders and patients of dosing errors associated with compounded injectable semaglutide products

Unsure how many units your restart dose is? Ask before you inject, as the FDA advises; at NexLife, call or text (949) 818-8000, 24/7.

“FDA encourages patients to talk with their health care provider or compounder about how to measure and administer the intended dose of compounded semaglutide.”

At NexLife

How do you restart semaglutide with NexLife?

In three steps. Nothing is prescribed until a licensed provider has looked at your break, so bring the details.

  1. Bring your vial’s label and your last dose date

A photo of your last vial’s label: the medicine, its strength in mg/mL and your dose in milligrams. Note the date you took your last dose. Paying less at another licensed provider? Add a receipt from the last 60 days for our Price Match Guarantee†.

  2. Tell us about the break

The health questions ask why you stopped, and about side effects, new medicines and any planned surgery. Your provider decides whether semaglutide is right for you now, and the dose you restart at.

  3. Restart with a new order

If you’re prescribed, it ships free and expedited, most often within 1 to 2 business days. Then it’s $119 a month on the 12-month plan, or $139 month to month, at every dose. Questions come to us 24/7.

Coming back after cancelling? You’ll complete a new online assessment, and a licensed provider reviews it.

I missed 2 weeks of semaglutide. What now?

After missing 2 or more weekly doses in a row, the Wegovy label (revised June 2026) says to restart the dose steps at a lower dose. Its patient guide says to contact your provider about how to restart, so talk with them before your next dose. They set the dose and how fast you step back up.

What dose should I restart semaglutide at after a month off?

Your provider decides. A month off is about 4 missed weekly doses, so the label’s rule for 2 or more applies: restart the dose steps at a lower dose. The label doesn’t name the dose; its steps are 0.25, 0.5, 1 and 1.7 mg, then 2.4 mg.

Do I start over at 0.25 mg after a break?

Not necessarily. The Wegovy label says to restart at a lower dose after 2 or more missed doses, but it doesn’t say which; 0.25 mg is its starting dose for new treatment. Your provider decides, based on how long you stopped and how you felt on your last dose.

I’m 3 days late on my semaglutide dose. Can I still take it?

Yes, by the Wegovy label’s rule. Three days late, your next dose is 4 days away, more than 2, so take it now and keep your usual day. If your next dose were less than 2 days away, you’d skip it.

Does weight come back if I pause semaglutide?

Some of it can. In the Wegovy label’s Study 5, people moved to placebo after 20 weeks gained 6.9% on average by week 68, while those who stayed on 2.4 mg lost another 7.9%. The trial studied FDA-approved Wegovy; compounded semaglutide wasn’t studied. Ask your provider before a planned pause.

Can I restart semaglutide after surgery?

Yes, when your provider says so. The Wegovy label notes rare reports of food getting into the lungs during anesthesia or deep sedation in people taking GLP-1 medicines, so tell everyone treating you before a planned procedure. If you stopped for 2 or more weeks, the label’s restart rule applies: back on the dose steps, at a lower dose.

Can I change my semaglutide injection day?

Yes. Wegovy’s patient guide allows a new day of the week as long as your last dose was 2 or more days before. The Ozempic label sets the same minimum: at least 2 days (48 hours) between doses.

Is my opened semaglutide vial still good after a break?

Only within 28 days of its first use, and before the beyond-use date on its label. The FDA says to throw out an opened multi-dose vial after 28 days, even with medicine left. A new vial may have a different strength, so check its mg/mL before you draw your restart dose.

How soon can I get semaglutide again through NexLife?

Once a licensed provider prescribes, our partner pharmacy usually ships compounded semaglutide in 1 to 2 business days, with free expedited shipping. If you cancelled before, you first complete a new 3-minute online assessment.

Does a 0.25 mg semaglutide restart cost less?

Not on Wegovy’s self-pay pen: Novo Nordisk lists it at $349 a month from 0.25 mg to 2.4 mg (checked November 1, 2026), though new NovoCare patients pay $199 for each of their first two fills (through Dec. 31, 2026). At NexLife, compounded semaglutide is $119 a month on the 12-month plan, or $139 month to month, at every dose. It isn’t FDA-approved; Wegovy is.

Why do you restart semaglutide at a lower dose?

To spare your stomach. The Wegovy label says stomach side effects came most often while the dose was going up, so after 2 or more missed weeks it restarts the dose steps at a lower dose. In its weight-loss trials, 73% of adults on Wegovy had stomach side effects, against 47% on placebo.

What is the 5-day rule for a missed semaglutide dose?

It’s the Ozempic label’s rule, for type 2 diabetes: take a missed dose within 5 days, or skip it once more than 5 days have passed. Wegovy’s rule looks ahead: take it if your next dose is more than 2 days away. Weekly, both cut off about 5 days after the missed dose.

How many units is a 0.25 mg semaglutide restart dose?

It depends on your vial’s strength: 0.25 mg is 10 units on a U-100 syringe from a 2.5 mg/mL vial, or 5 units from a 5 mg/mL vial. Units equal the dose in mg divided by the strength in mg/mL, times 100, so check the strength on every new vial.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
