By the NexLife Medical Team · Medically reviewed by Adam Kennah, MD · Updated
Keep compounded semaglutide in the fridge at 36°F to 46°F, in its box, unless your label says otherwise, and never let it freeze (checked November 1, 2026). Use an opened vial within 28 days or by its beyond-use date, whichever comes first.
- Wegovy’s pen rules aren’t your vial’s rules
- A vial’s life is set by days, not by what’s left

Step by step
How do you store compounded semaglutide?
Six steps, from the day it arrives to the day you travel. If your label says something different, follow your label.
- Read your label first. The storage directions and beyond-use date on your prescription label are the ones to follow.
- Refrigerate it, in its box. Keep it at 36°F to 46°F (2°C to 8°C), unless your label says otherwise. The box protects it from light.
- Never let it freeze. Don’t use a vial that has frozen.
- Count 28 days from the first puncture. Use an opened vial within 28 days, or by its beyond-use date if that’s sooner, even if medication is left.
- Fly with it in your carry-on. Use an insulated case with a cold pack, declare it at screening, and put it back in a fridge when you arrive.
- Warm, frozen or cloudy? Don’t use it. Keep the vial and the packaging, and call or text NexLife at
(949) 818-8000 , 24/7. We contact the pharmacy for you.
The same six steps, from delivery to travel. Your prescription label and your prescriber’s instructions come first.

Do Wegovy’s storage rules apply to your vial?
It’s easy to borrow Wegovy’s storage rules, because they’re printed everywhere. They don’t transfer. Here’s why, side by side.
| Wegovy single-dose pen | Compounded vial | |
|---|---|---|
| Who sets the rules | Novo Nordisk, from stability testing reviewed by the FDA | Your prescription label |
| Out of the fridge | Up to 28 days at 46°F to 86°F, before the cap comes off | Only if your label allows it |
| After first use | Single-use pen, then discarded | Up to 28 days, or the beyond-use date if sooner |
| Freezing | Don’t freeze; don’t use if frozen | Don’t freeze; don’t use if frozen |

Which storage rules apply to compounded semaglutide?
Compounded semaglutide comes with its own storage rules, printed on your prescription label. Here’s how to follow them, and who to call: us.
- Your label wins. The storage directions and beyond-use date on your label are the ones to follow.
- Pens aren’t vials. Wegovy pen rules don’t carry over to a compounded vial.
- Weather happens. A hot porch or a frozen stoop can change what you should do next.
One number to call. We handle the rest.
- Something looks off? Call or text us first. We follow up with the pharmacy for you.
- Shipped fast. Our partner pharmacy usually ships within 1 to 2 business days, discreetly packed.
- $119 a month on the 12-month plan, at every dose. Semaglutide from 0.25 mg to 2.4 mg, or $139 month to month, with no membership.

Opened vials
How long does an opened vial of compounded semaglutide last?
Days, not doses, set the limit. Once you puncture a multiple-dose vial, the clock starts. USP Chapter 797, the sterile-compounding standard, calls for discarding it after 28 days, even if there’s medication left, because every puncture adds a little contamination risk.
Your dose decides how much you use. Early on, at low doses like 0.25 mg a week, a vial may hold more than four weeks of medication. That extra doesn’t make it safe to stretch past day 28.
Concentration changes the math. Compounded vials don’t all hold the same strength, so the same dose can be a different volume in a different vial. Go by the milligrams and the instructions your provider gives you, not by units you used before.
“Discard the vial within 28 days after the first use, even if medication remains in the vial.”
U.S. Food and Drug Administration, FDA’s Concerns with UnapprovedGLP-1 Drugs Used for Weight Loss

Vial math
How many units is your semaglutide dose?
It depends on your vial’s strength. Here are the Wegovy label’s dose steps at two example strengths.
| Weeks | Weekly dose | At 2.5 mg/mL | At 5 mg/mL |
|---|---|---|---|
| 1 to 4 | 0.25 mg | 0.1 mL (10 units) | 0.05 mL (5 units) |
| 5 to 8 | 0.5 mg | 0.2 mL (20 units) | 0.1 mL (10 units) |
| 9 to 12 | 1 mg | 0.4 mL (40 units) | 0.2 mL (20 units) |
| 13 to 16 | 1.7 mg | 0.68 mL (68 units) | 0.34 mL (34 units) |
| 17 onward | 2.4 mg | 0.96 mL (96 units) | 0.48 mL (48 units) |
The math: mL = dose ÷ strength (mg per mL); units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.
Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. If your label’s units differ from this chart, follow the label and ask your pharmacy. Every step: the semaglutide dosage chart.
On the road
How do you travel with semaglutide?
Pack it in your carry-on
Cargo holds can get cold enough to freeze a vial, and checked bags get lost. Keep it with you.
Use an insulated case and a cold pack
TSA allows medically necessary gel ice packs in reasonable amounts, even when they’re melted or slushy.
Tell the officer at screening
TSA allows larger amounts of medically necessary liquids. Declare the vial and your supplies at the checkpoint.
Back in a fridge on arrival
Hotel minibars can run warm or freeze things. Ask for a medical fridge if the room has none.

- No. You pay the same at every dose.
- What if I’m not prescribed?
- You aren’t charged for medication.
- Can I cancel?
- Yes, by email. It takes effect 30 days after we receive it. Plans renew until you cancel; our cancellation policy has the details.
- Do I need insurance?
- No. Pay with an HSA or FSA card, or with Affirm, Klarna or Afterpay at checkout.
How should I store compounded semaglutide?
In the fridge at 36°F to 46°F (2°C to 8°C), in its box, and never frozen, unless your prescription label says otherwise. The storage line and beyond-use date on your label are the rules for your vial. Once it’s opened, use it within 28 days, or by that date if sooner.
Can compounded semaglutide be left out of the fridge?
Only if your label says so, and only for as long as it says. Wegovy’s single-dose pen can be kept at 46°F to 86°F for up to 28 days before the cap comes off, but that was tested for the pen, not for a compounded vial. Unsure? Ask the program that prescribed it.
How long is a vial of compounded semaglutide good for after opening?
Up to 28 days after you first puncture it, or until the beyond-use date on the label if that’s sooner. Then discard it, even if medication is left. That’s the USP Chapter 797 standard for multiple-dose vials, because every puncture adds a little contamination risk.
Can I fly with compounded semaglutide?
Yes. Keep it in your carry-on, never in checked bags, in an insulated case with a cold pack. TSA allows medically necessary liquids in larger amounts, and medically necessary gel ice packs even when they’re melted or slushy. Tell the officer at screening, and put it back in a fridge when you arrive.
What if my semaglutide arrived warm or froze?
Don’t use it. Keep the vial and the packaging, and contact the program that prescribed it. At NexLife, call or text
Does compounded semaglutide need to stay in its box?
Yes, unless your label says otherwise. Keep the vial in its original box in the fridge, at 36°F to 46°F: the box protects it from light and from knocks. Your label’s storage line is the rule for your vial, so read it again whenever a new vial arrives.
What temperature should compounded semaglutide be stored at?
36°F to 46°F (2°C to 8°C), in the fridge and in its box, unless your label says otherwise. Never let it freeze. Wegovy’s single-dose pen allows up to 86°F for up to 28 days, but that rule was tested for the pen, not for a compounded vial.
How do I know if my compounded semaglutide has gone bad?
Don’t use it if it froze, looks cloudy, has particles, arrived warm or damaged, or is past 28 days from opening or its beyond-use date. Call or text NexLife at (949) 818-8000 instead. We contact the pharmacy and arrange what comes next.
Can I keep semaglutide in a hotel fridge?
Yes, but check the fridge first. Hotel minibars can run warm or freeze things, so ask for a medical fridge if the room has none. Keep the vial at 36°F to 46°F in its box, unless your label says otherwise. Never let it freeze.
What if I run out of semaglutide before day 28?
Tell the program that prescribed it early. At NexLife, tell us early and we time your next shipment around your dose. Call or text (949) 818-8000, 24/7: real people answer, and we follow up with the pharmacy for you, so you never have to chase anyone.
Important safety information
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other
Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.
