Pharmacies

Compounding pharmacies licensed to mail compounded injections to New Yorkers

Only pharmacies registered with the New York State Education Department may mail you compounded semaglutide. A pharmacy outside New York must register with the State Education Department as a nonresident establishment ($345 application fee) and stay in good standing at home. It must keep records of drugs shipped here, put a toll-free number on each label and name a New York agent. Outsourcing facilities must send inspection results yearly.

  • Check it in a minute. Look up any pharmacy on New York’s pharmacy license lookup before you pay.
  • Match your label. The pharmacy’s name and address on your box should match its New York registration.
  • NexLife’s partner pharmacies. Absolute Pharmacy (Lutz, FL) and Boudreaux’s New Drug Store (Lake Charles, LA). We send New York prescriptions only to the ones with the licenses New York requires.

“Generally, state boards of pharmacy have primary responsibility for the day-to-day oversight of state-licensed pharmacies that are not registered with FDA as outsourcing facilities.”

Can you bring semaglutide back from Canada?

Not legally, in most cases. Buying in Canada? The FDA says drugs approved in another country are unapproved in the U.S., and importing them, for personal use or otherwise, violates federal law. Heat is a second risk: Zepbound’s and Wegovy’s labels don’t allow storage above 86°F. The legal route is a New York-licensed prescriber and a registered pharmacy.

How many units is your semaglutide dose?

It depends on your vial’s strength. Here are the Wegovy label’s dose steps at two example strengths, with the math to check your own.

Semaglutide dose steps from the Wegovy label, with the volume and units on a U-100 syringe at two example strengths, 2.5 mg/mL and 5 mg/mL; your vial’s label decides
WeeksWeekly doseAt 2.5 mg/mLAt 5 mg/mL
1 to 40.25 mg0.1 mL (10 units)0.05 mL (5 units)
5 to 80.5 mg0.2 mL (20 units)0.1 mL (10 units)
9 to 121 mg0.4 mL (40 units)0.2 mL (20 units)
13 to 161.7 mg0.68 mL (68 units)0.34 mL (34 units)
17 onward2.4 mg0.96 mL (96 units)0.48 mL (48 units)

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

Under criteria New York’s Medicaid drug review board approved on Feb. 27, 2025, it can cover Wegovy with prior authorization for cardiovascular disease, such as a past heart attack or stroke, with a BMI of 40 or more and heart-healthy lifestyle changes, for up to two tries per lifetime.

Does New York Medicaid cover Ozempic for weight loss?

No. NYRx names Ozempic among the GLP-1s excluded for weight-loss use, and it says weight loss has never been a Medicaid-covered use. Ozempic is covered for type 2 diabetes: the Ozempic pen is a preferred drug, with clinical criteria, on the NYRx quick list revised Oct. 22, 2026.

Does the Empire Plan cover Wegovy?

Yes, with prior authorization. The Empire Plan’s October 2026 drug list shows Wegovy as a preferred anti-obesity drug that needs prior authorization, as are Zepbound and Foundayo. The list doesn’t spell out the approval criteria, so ask the plan’s prescription program what your prescriber must send before you fill it.

Is compounded semaglutide legal in New York?

Yes, with a prescription written for you and filled by a pharmacy New York has registered. The FDA ended the semaglutide shortage on Feb. 21, 2025, and its compounding grace periods ended April 22 and May 22, 2025. A compounded version now counts as a copy of Wegovy unless your prescriber documents a significant difference for you. It isn’t FDA-approved.

Does my semaglutide prescriber need a New York license?

Yes. New York requires full licensure and current registration of anyone who practices in the state, whether you’re seen in person or by telehealth. New York isn’t in the Interstate Medical Licensure Compact; bills to join (S1505 and A1983) are still in committee. Check any provider’s license free on the State Education Department’s online search.

Can an out-of-state pharmacy ship semaglutide to New York?

Yes, once it registers with New York. A pharmacy outside the state must register with the State Education Department as a nonresident establishment, pay a $345 application fee, stay in good standing at home and print a toll-free number on each label. It must also name a resident agent in New York.

How do I keep semaglutide cold in a New York summer?

Refrigerate it as soon as it arrives. Before the cap comes off, Wegovy’s label allows 46°F to 86°F for up to 28 days, and says to discard a pen that got hotter. Compounded vials follow their own pharmacy label. In summer 2026, Stewart Field near Newburgh hit 86°F or more on 24 days.

How much does semaglutide cost in New York without insurance?

Compounded semaglutide runs $119 to $299 a month online, median $199 (price index, checked Nov. 1, 2026). The Wegovy pen at NovoCare Pharmacy is $199 for each of a new patient’s first two fills (through Dec. 31, 2026), then $349 a month ($399 at 7.2 mg); the Wegovy pill is $149 to $299. NexLife charges $119 a month on the 12-month plan.

Where can I get semaglutide in Buffalo or Rochester?

You don’t need a local clinic. A New York-licensed provider can treat you online, and a pharmacy registered with New York can ship to Buffalo, Rochester or anywhere upstate. Summers are milder there: Buffalo’s airport hit 86°F or more on 9 days in 2026. Still, get the box into the fridge the day it arrives.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product.

Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.

How we checked. On Nov. 1, 2026, we read each program’s public pages (no quiz, account or checkout); on Nov. 2, 2026, we read New York’s Medicaid, licensing, telehealth and pharmacy rules at their sources. The heat counts are ours, from NOAA’s daily station data at 11 weather stations.

How we make money. We earn revenue when people join NexLife, one of the programs here, so we name the catch for every option, ours included.
