Pharmacies

Compounding pharmacies licensed to mail compounded injections to Texans

Only pharmacies licensed by the Texas State Board of Pharmacy may mail you compounded semaglutide. A pharmacy outside Texas needs a Texas Class E (nonresident) license to ship prescriptions here, and its pharmacist-in-charge must hold a Texas license. If it compounds sterile drugs, such as injections, it needs a Class E-S license, which the Board won’t issue or renew without an inspection by the Board or its designee.

How it works

How to get semaglutide in Texas, from your couch to your door

Online, in four steps, with no insurance needed. Here’s what happens at NexLife.

  1. Answer a few health questionsAbout 3 minutes online: your health history, your medicines and your goals.
  2. A licensed provider reviews themA provider licensed to treat you in Texas decides whether compounded semaglutide is right for you, following Texas’s rules. You’re prescribed only if it is.
  3. A licensed pharmacy prepares itYour prescription goes to a partner pharmacy with the licenses Texas requires, which usually ships within 1 to 2 business days, free.
  4. It comes to your doorIn an insulated, cold-packed box. Put it in the fridge right away; if it arrives warm, don’t use it: call or text (949) 818-8000.

How do you keep semaglutide cool in a Texas summer?

Store compounded semaglutide the way your prescription label says, usually in the fridge at 36°F to 46°F, and get the box inside fast. Wegovy’s rule for pens kept at 46°F to 86°F for up to 28 days doesn’t apply to compounded vials. Orders through NexLife ship in insulated, cold-packed boxes; if yours arrives warm, don’t use it: call or text (949) 818-8000.

Days with a high of 86°F or more, June 1 to Sept. 30, 2026, at 12 NOAA weather stations in and near Texas (ACIS daily data; our count). Tyler misses 1 day of data; Port Arthur misses 1 day of data.

On hot days, be home for delivery or pick a shaded drop-off spot, and get the box into the fridge right away. In a disaster, if your prescriber can’t be reached, a Texas pharmacist may refill most prescriptions with up to a 72-hour supply when a gap could interrupt treatment. After a governor’s disaster declaration, once the Board of Pharmacy notifies pharmacies, the limit is a 30-day supply. (Texas Legislature) More: how to store compounded semaglutide.

Can you get semaglutide online in Texas?

Yes. Texas lets licensed providers treat you by telehealth, under the rules below. Your prescriber needs a Texas license, and a pharmacy outside Texas needs a Texas license to ship to you.

State law

Getting care online

Texas holds online care to the in-person standard of care. Before treating you, a practitioner must get your informed consent and use the clinical information that standard requires, such as your history, labs and prescription history. Then they must give follow-up guidance and, if you agree, send your primary care doctor a report within 72 hours.

Does insurance cover semaglutide in Texas in 2026?

For weight loss alone, rarely. Texas Medicaid covers Wegovy only for heart disease or MASH, not for weight loss alone. Texas’s state employee plan doesn’t cover GLP-1s for weight loss. With Medicare Part D, the GLP-1 Bridge can bring the Wegovy pen or pill to $50 a month.

Texas Medicaid: not for weight loss

Not for weight loss alone. Since Dec. 27, 2024, Texas Medicaid has covered Wegovy with prior authorization for adults 45 and older with cardiovascular disease who are overweight or obese. Can you bring semaglutide back from Mexico?

Not legally, in most cases. In most cases, it’s illegal to bring medicines from Mexico for personal use, the FDA says, and drugs approved there are unapproved in the U.S. The FDA is also aware of counterfeit Ozempic marketed in the U.S. Heat adds risk: McAllen’s airport hit 100°F or more on 97 days in summer 2026, and the Zepbound and Wegovy labels cap unrefrigerated storage at 86°F.

“Importation of unapproved drugs, whether for personal use or otherwise, violates federal law.”

How many units is your semaglutide dose?

It depends on your vial’s strength. Here are the Wegovy label’s dose steps at two example strengths, with the math to check your own.

Semaglutide dose steps from the Wegovy label, with the volume and units on a U-100 syringe at two example strengths, 2.5 mg/mL and 5 mg/mL; your vial’s label decides
WeeksWeekly doseAt 2.5 mg/mLAt 5 mg/mL
1 to 40.25 mg0.1 mL (10 units)0.05 mL (5 units)
5 to 80.5 mg0.2 mL (20 units)0.1 mL (10 units)
9 to 121 mg0.4 mL (40 units)0.2 mL (20 units)
13 to 161.7 mg0.68 mL (68 units)0.34 mL (34 units)
17 onward2.4 mg0.96 mL (96 units)0.48 mL (48 units)

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

NexLife keeps one price at every dose with care included ($119 a month on the 12-month plan or $139 month to month), PreventiveMD has the lowest month-to-month compounded price we found ($119), Ondra Health the lowest 12-month price ($96 a month, paid upfront), and NovoCare Pharmacy sells FDA-approved Wegovy, from $149 a month for the pill. NexLife publishes this guide.

Does Texas Medicaid cover Wegovy for weight loss?

Not for weight loss alone. Since Dec. 27, 2024, Texas Medicaid has covered Wegovy with prior authorization for adults 45 and older with cardiovascular disease who are overweight or obese. Since Nov. 24, 2025, traditional Medicaid also covers adults with the liver disease MASH, without cirrhosis, and moderate to advanced fibrosis.

Does HealthSelect cover Wegovy for Texas state employees?

Only to lower heart risk. HealthSelect’s prescription plan may cover Wegovy, with prior authorization, for adults with established cardiovascular disease who are overweight or obese. It excludes drugs prescribed only for obesity (plan document effective Sept. 1, 2025). GLP-1s for type 2 diabetes are covered with prior authorization.

Do TRS-ActiveCare or UT SELECT cover Wegovy?

No. TRS says it doesn’t cover weight-loss drugs under any circumstance and names Wegovy as an example, though it covers this drug class for diabetes. UT SELECT’s drug plan for Sept. 1, 2026, to Aug. 31, 2027, excludes GLP-1s for conditions other than diabetes, so heart risk doesn’t qualify there either.

Is compounded semaglutide legal in Texas?

It can be, when it’s compounded for you. The FDA declared the semaglutide shortage over on Feb. 21, 2025, so copies of Wegovy can’t be made regularly unless your prescriber documents a significant difference for you. Pharmacies compounding injections for Texans must follow the state’s sterile compounding rule, updated March 1, 2026. It isn’t FDA-approved.

Can a nurse practitioner prescribe semaglutide in Texas?

Yes, under a physician agreement. Texas nurse practitioners and physician assistants prescribe through a written prescriptive authority agreement with a supervising physician. The agreement must be reviewed yearly, and they must hold documented meetings at least once a month. They also need an active Texas license.

Can a pharmacy in another state ship semaglutide to Texas?

Yes, if it holds a Texas license. Out-of-state pharmacies need a Class E license from the Texas State Board of Pharmacy, or a Class E-S license if they compound sterile drugs such as injections. A Class E-S license isn’t issued or renewed without an inspection by the Board or its designee. You can check any pharmacy online.

How do I keep semaglutide safe in Texas heat?

Refrigerate it as soon as it arrives. A Wegovy single-dose pen can stay out of the fridge for up to 28 days before its cap comes off, but only up to 86°F. In summer 2026, Houston’s main airport hit 86°F or more on 116 of 122 days and San Antonio’s on 118. Compounded vials follow their own pharmacy label.

How much does semaglutide cost in Texas without insurance?

Online, compounded semaglutide costs $119 to $299 a month, median $199 (checked Nov. 1, 2026). The Wegovy pen at NovoCare Pharmacy is $199 for each of a new patient’s first two fills (through Dec. 31, 2026), then $349 a month ($399 at 7.2 mg); Wegovy pills cost $149 to $299. NexLife charges $119 a month on the 12-month plan or $139 month to month.

Where can I get semaglutide in San Antonio or Austin?

You can get it without leaving home. A provider licensed in Texas can treat you online, and a pharmacy the Texas State Board of Pharmacy has licensed can ship it to San Antonio, Austin or anywhere else in Texas. Before you visit a clinic or med spa, look up the prescriber’s license on the state board’s website.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product.

Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
