Only Wegovy is FDA-approved. Compounded semaglutide costs less and is prepared by licensed 503A pharmacies.

  • Coverage is limited. Many plans exclude weight-loss medicines, so the cash price usually decides.

Before you buy

What should a compounded semaglutide label say?

One check before you buy compounded semaglutide: the label should say semaglutide, not a salt form such as semaglutide sodium or acetate. The FDA says it isn’t aware of any lawful basis to compound salt forms. What’s in compounded semaglutide.

“These salt forms, including semaglutide sodium and semaglutide acetate, are different active ingredients than are used in the approved drugs.”

Your call

Which fits you better, Wegovy or compounded semaglutide?

Wegovy may fit better if…

  • Your insurance covers it, or Medicare’s GLP-1 Bridge does
  • You want the FDA-approved medicine, including its heart-risk benefit for people with heart disease
  • You’d prefer a daily pill to a weekly shot

Compounded may fit better if…

  • You’re paying cash and want one price at every dose: $119 a month at NexLife on the 12-month plan
  • Your provider determines it offers a significant difference for you
  • You’re comfortable drawing your dose from a vial

“Patients should obtain a prescription from their doctor and fill the prescription at a state-licensed pharmacy.”

Your provider decides which is right for you.

Step by step

How do you choose between compounded semaglutide and Wegovy?

Coverage comes first, because many plans exclude weight-loss medicines. Then weigh FDA approval, the evidence, the price and the form, with your provider.

  1. Check your coverage. If your insurance covers Wegovy, it may fit better. Eligible Part D enrollees can get it through Medicare’s GLP-1 Bridge at a $50 monthly copay, with prior authorization; the Bridge doesn’t cover compounded semaglutide.
  2. Compare FDA status. Wegovy is FDA-approved and was studied in the STEP trials. Compounded semaglutide isn’t FDA-approved, and the FDA doesn’t review it for safety, effectiveness or quality before it’s sold.
  3. Look at the trial evidence. Compounded semaglutide hasn’t been tested in clinical trials. In STEP 1, people on Wegovy lost 14.9% of their body weight on average over 68 weeks; that’s an average for Wegovy, not a promise.
  4. Compare the cash price. Wegovy is $149 to $399 a month through NovoCare, by form and dose (checked November 1, 2026). Compounded semaglutide at NexLife is $119 a month on the 12-month plan or $139 month to month, at every dose.
  5. Pick the form, and read the label. Wegovy is a weekly pen or a daily pill; compounded semaglutide is a weekly injection drawn from a vial. Its label should say semaglutide, not a salt form such as semaglutide sodium or acetate.
  6. Let your provider decide. Your provider reviews your history and goals, and prescribes compounded semaglutide only when it offers a significant difference for you. Switching from Wegovy is a change of product, not a refill, so your provider decides where you start.

Your answers to steps 1 to 5 shape the choice; at step 6, your provider decides which is right for you.

Vial math

How many units is your semaglutide dose?

It depends on your vial’s strength. Here are the Wegovy label’s dose steps at two example strengths.

Semaglutide dose steps from the Wegovy label, with the volume and units on a U-100 syringe at two example strengths, 2.5 mg/mL and 5 mg/mL
WeeksWeekly doseAt 2.5 mg/mLAt 5 mg/mL
1 to 40.25 mg0.1 mL (10 units)0.05 mL (5 units)
5 to 80.5 mg0.2 mL (20 units)0.1 mL (10 units)
9 to 121 mg0.4 mL (40 units)0.2 mL (20 units)
13 to 161.7 mg0.68 mL (68 units)0.34 mL (34 units)
17 onward2.4 mg0.96 mL (96 units)0.48 mL (48 units)

The math: mL = dose ÷ strength (mg per mL); units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

Is compounded semaglutide the same as Wegovy?

No. Wegovy is Novo Nordisk’s FDA-approved semaglutide, sold as a weekly pen or a daily pill, and studied in clinical trials. Compounded semaglutide is prepared by a state-licensed pharmacy for your prescription. It isn’t FDA-approved, and it hasn’t been studied in trials. Its label should say semaglutide, not a salt form.

Is compounded semaglutide the same as Ozempic?

No. Ozempic is Novo Nordisk’s FDA-approved semaglutide injection for type 2 diabetes. Compounded semaglutide is prepared by a state-licensed pharmacy for your prescription, and it isn’t FDA-approved. To see how Novo Nordisk’s two semaglutide injections differ from each other, read Ozempic vs Wegovy.

Does compounded semaglutide work as well as Wegovy?

No one can say for sure: compounded semaglutide hasn’t been tested in clinical trials. FDA-approved Wegovy has. In STEP 1, people on Wegovy lost 14.9% of their body weight on average over 68 weeks. That result applies to Wegovy, not to compounded semaglutide, and an average isn’t a promise for anyone.

Is the Wegovy pill cheaper than compounded semaglutide?

It depends on your dose. Through NovoCare, the Wegovy pill is $149 a month at its 1.5 mg starting dose and $199 to $299 at higher doses (checked November 1, 2026). Compounded semaglutide at NexLife is $119 a month on the 12-month plan, or $139 month to month, at every dose.

Can I switch from Wegovy to compounded semaglutide?

Often, yes, after a provider’s review. It’s a change of product, not a refill, so your provider decides whether it fits you and where you start. Compounded semaglutide is prescribed only when it offers a significant difference for you. For doses and units, see Wegovy to compounded semaglutide.

How much is Wegovy without insurance?

$149 to $399 a month through NovoCare, by form and dose (checked November 1, 2026). The pill is $149 at 1.5 mg, $199 at 4 mg and $299 at 9 and 25 mg. The pen is $199 for each of a new patient’s first two fills (through Dec. 31, 2026), then $349 a month, or $399 at 7.2 mg. More: semaglutide without insurance.

Is the Wegovy pill as effective as the shot?

There’s no head-to-head answer: they were tested in separate trials. In its own trial, the 25 mg pill led to 13.6% average weight loss at 64 weeks. In STEP 1, the 2.4 mg shot led to 14.9% at 68 weeks. Averages aren’t promises, and your provider can help you choose.

Does Medicare cover Wegovy?

Yes, for eligible Part D enrollees, through Medicare’s GLP-1 Bridge: Wegovy for weight loss at a $50 monthly copay, with prior authorization, through December 31, 2027. That’s $600 to $650 for a first year. The Bridge doesn’t cover compounded semaglutide. Details: Medicare GLP-1 coverage.

Is compounded semaglutide FDA-approved?

No. Compounded semaglutide isn’t FDA-approved, and the FDA doesn’t review it for safety, effectiveness or quality before it’s sold. Wegovy is the FDA-approved brand-name semaglutide for weight loss. A licensed provider prescribes compounded semaglutide only when it offers a significant difference for you, and a state-licensed pharmacy prepares it.

Which is better, Wegovy or compounded semaglutide?

Neither is right for everyone. Wegovy may fit better if your insurance or Medicare’s GLP-1 Bridge covers it, or if you want the FDA-approved medicine. Compounded semaglutide may fit if you’re paying cash and your provider finds it offers a significant difference for you. At NexLife, it’s $119 a month on the 12-month plan.

Is compounded semaglutide safe?

The FDA hasn’t checked: it doesn’t review compounded semaglutide for safety, effectiveness or quality before it’s sold, and it hasn’t been studied in trials. The FDA advises getting a prescription and filling it at a state-licensed pharmacy. Check the label, too: it should say semaglutide, not a salt form.

Is compounded semaglutide a generic version of Wegovy?

No. A generic drug is approved by the FDA and must show therapeutic equivalence to a brand-name drug, while a compounded drug isn’t FDA-approved. Compounded semaglutide is prepared by a state-licensed pharmacy for your prescription, and it hasn’t been studied in trials.

Does insurance cover compounded semaglutide?

Coverage is limited. Many plans exclude weight-loss medicines, so the cash price usually decides, and Medicare’s GLP-1 Bridge doesn’t cover compounded semaglutide. At NexLife, no insurance is needed: pay with an HSA or FSA card, or with Affirm, Klarna or Afterpay.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
