    • Changed: Belle shows compounded semaglutide from $93 a month, with no plan length shown, so it stays below the tables.
    • Embody stays at its regular prices, $349 and $299; its one-month pages show a lower sale price. The medians stay $237 for tirzepatide across 14 programs and $199 for semaglutide across 17. No change in NexLife’s prices: $139 and $119 a month on 12 months, or $169 and $139 month to month, at every dose.
    • Updated: the data file, now nexlife-glp-1-price-index-2026-11-01.csv.
  3. October 19, 2026

    • Removed: one compounded program, from both tables.
    • Updated: Embody is listed at its regular month-to-month prices, $349 for tirzepatide and $299 for semaglutide; its first-month sale and prepaid bundles are no longer listed. Its Trustpilot rating: 3.9 out of 5, from 11,253 reviews.
    • The index now covers 21 online programs. The medians are $237 for tirzepatide across 14 programs and $199 for semaglutide across 17. No change in NexLife’s prices: $139 and $119 a month on 12 months. For each medicine, its month-to-month price ($169 and $139, at every dose) is below the index’s median month-to-month price.
  4. October 18, 2026

    • Added: MEDVi’s compounded semaglutide price. Its GLP-1 page lists $179 for the first month, labeled a fall sale, then $299 a month for refills, so the index lists $299 month to month with the first month beside it. It lists no compounded tirzepatide price, so MEDVi stays below the tirzepatide table.
    • Confirmed: Mochi Health’s membership on longer plans: $69 a month on 3 months, $59 on 6 and $49 on 12, on top of medication priced by dose.
    • The semaglutide median moved to $187 across 18 programs, from $175 across 17. No change in NexLife’s prices: $139 and $119 a month on 12 months, or $169 and $139 month to month, at every dose.
  5. October 15, 2026

    • Re-checked every price on each provider’s own website.
    • Added: Noom, which publishes compounded tirzepatide at $149 to start, then $299 a month, and semaglutide at $129, then $279, each billed every 12 weeks. Also added to the brand-name prices: Foundayo, Lilly’s GLP-1 pill, and Medicare’s GLP-1 Bridge.
    • Moved into the tables: Ivím Health, which now lists medication from $117 (tirzepatide) and $65 (semaglutide) a month, plus a $75 monthly membership.
    • Moved out of the tables: Belle. Its pages now show only starting prices ($179 and $109 a month) and no month-to-month price, so it’s listed below the tables until we can confirm one.
    • Changed: CoreAge Rx now advertises prices from $129 for tirzepatide and $79 for semaglutide, down from $149 and $99. Shapely’s $99 membership is $49 for the first month.
    • The medians moved to $225 for tirzepatide and $175 for semaglutide, from $237 and $187, because of the programs added and moved. No change in NexLife’s prices: $139 and $119 a month on 12 months, or $169 and $139 month to month, at every dose.
  6. October 13, 2026

    • Re-checked Embody’s and PreventiveMD’s prices, plans and terms on their own websites.
    • Updated: Embody’s product pages label its first-month prices as one-month sales, with regular prices of $299 (semaglutide) and $349 (tirzepatide), and its terms describe a reduced first-month price followed by standard monthly pricing from month two. Under our rule for first-month offers, the index lists $299 and $349 as Embody’s month-to-month prices.
    • Updated: PreventiveMD’s compounded tirzepatide is $139 a month, month to month. The October 8 version listed a higher tirzepatide price, now corrected. Its semaglutide is unchanged: $119 a month.
    • NexLife’s month-to-month prices stayed among the most affordable in this index. No change in NexLife’s prices.
  7. October 8, 2026

    • Re-checked every price on each provider’s own website on October 7. Embody’s and PreventiveMD’s tirzepatide prices were updated on October 8.
    • Listed Embody’s compounded tirzepatide at a sale price (corrected on October 13: its regular price is $349); its semaglutide’s regular price is $299.
    • Listed PreventiveMD’s tirzepatide at a higher month-to-month price (corrected on October 13: $139), and its semaglutide at $119 a month.
    • Removed GobyMeds (its pricing page no longer lists GLP-1 prices), TrimRx (its pricing pages didn’t load) and Sesame (its program fee couldn’t be confirmed).
    • NexLife’s month-to-month prices were among the most affordable in this index for tirzepatide and for semaglutide. No change in NexLife’s prices. NexLife matches a lower regular price from another licensed U.S. provider.†
  8. October 6, 2026

    • Re-checked every price on each provider’s own website and expanded the index from 10 providers (6 compounded, 4 brand-name only) to 23 compounded providers and 7 brand-name sources.
    • Correction: earlier versions said NexLife had the lowest fixed monthly price of the 10 providers checked. With 13 more providers added, PreventiveMD publishes a lower tirzepatide price and AltRx advertises lower ones without stating a plan length.
    • Correction: Refills publishes tirzepatide at $249 a month ($159 on 12 months) and semaglutide at $199 ($99 on 12 months). Earlier versions said it listed no tirzepatide price.
    • Correction: Remedy Meds publishes $399 per 28 days for tirzepatide ($279 for the first month) and $299 for semaglutide ($179). Earlier versions said its price wasn’t listed.
    • AltRx moved to the list of providers without a published month-to-month price: it advertises $149 and $89 a month but doesn’t state the plan length.
  9. October 5, 2026

    • Page text and check dates updated. No prices changed.
  10. October 1, 2026

    • Re-checked the original 10 providers. Ro’s Wegovy range changed to $298 to $548 a month, from $348 to $498.

Vial math

How many units is your tirzepatide or semaglutide dose?

On a compounded vial, the units depend on its strength. Here are the label doses at example strengths, with the math to check your own.

Tirzepatide

Tirzepatide doses on Zepbound’s label, with the volume and units on a U-100 syringe at an example strength of 25 mg per mL; your vial’s label decides
DosemL at 25 mg/mLUnits (U-100)On the label
2.5 mg0.1 mL10 unitsStarting dose, for 4 weeks
5 mg0.2 mL20 unitsA maintenance dose
7.5 mg0.3 mL30 unitsA step up
10 mg0.4 mL40 unitsA maintenance dose
12.5 mg0.5 mL50 unitsA step up
15 mg0.6 mL60 unitsThe maximum, and a maintenance dose

The math: mL = dose ÷ strength (mg per mL), and units = mL × 100. At 25 mg/mL, 10 mg is 10 ÷ 25 = 0.4 mL, or 40 units.

Semaglutide

Semaglutide dose steps from the Wegovy label, with the volume and units on a U-100 syringe at two example strengths, 2.5 mg/mL and 5 mg/mL
WeeksWeekly doseAt 2.5 mg/mLAt 5 mg/mL
1 to 40.25 mg0.1 mL (10 units)0.05 mL (5 units)
5 to 80.5 mg0.2 mL (20 units)0.1 mL (10 units)
9 to 121 mg0.4 mL (40 units)0.2 mL (20 units)
13 to 161.7 mg0.68 mL (68 units)0.34 mL (34 units)
17 onward2.4 mg0.96 mL (96 units)0.48 mL (48 units)

The math: mL = dose ÷ strength (mg per mL); units = mL × 100. At 2.5 mg/mL, 1 mg is 1 ÷ 2.5 = 0.4 mL, or 40 units. At 5 mg/mL, the same 1 mg is 0.2 mL, or 20 units.

25 mg per mL is an example only: it’s the strength of the 15 mg multi-dose vial on Zepbound’s label (15 mg per 0.6 mL). Read the strength on your own vial’s label; your provider sets your dose, in steps at least 4 weeks apart. Every step: the tirzepatide dosage chart. Both strengths are examples only. The same dose is a different number of units at a different strength, so read the strength on your own vial’s label. Your prescription sets your dose and schedule, which may differ from these label steps. Every step: the semaglutide dosage chart.

What is the cheapest compounded tirzepatide online right now?

In our index, PreventiveMD’s $139 is the lowest month-to-month price, the same at every dose (checked November 2, 2026). Mondae, not in the tables yet, lists $126 on a 1-month plan (read November 1, 2026). NexLife is one of the most affordable: $139 a month on the 12-month plan or $169 month to month. Every plan length: cheapest tirzepatide.

What is the cheapest compounded semaglutide online right now?

In our index, PreventiveMD’s $119 is the lowest month-to-month price, the same at every dose (checked November 2, 2026). Mondae, not in the tables yet, lists $87 on a 1-month plan (read November 1, 2026). NexLife is one of the most affordable: $119 a month on the 12-month plan or $139 month to month. Every plan length: cheapest semaglutide.

What is the average (median) price of compounded tirzepatide and semaglutide?

The median month-to-month price, all in, is $237 for compounded tirzepatide across 14 programs and $199 for compounded semaglutide across 17, checked November 1, 2026. NexLife’s 12-month plans are $139 (tirzepatide) and $119 (semaglutide) a month at every dose; month to month, its $169 and $139 are $68 and $60 under those medians.

How much do compounded tirzepatide and semaglutide cost?

Compounded tirzepatide costs $139 to $399 a month at the online programs in this index (median $237), and compounded semaglutide $119 to $299 (median $199), all in, month to month. We checked every price on November 1, 2026. Longer plans paid upfront cost less per month.

How much does GLP-1 medication cost?

In this index, compounded GLP-1 medication costs $119 to $399 a month online, all in. Brand-name GLP-1s cost $149 to $449 a month at the manufacturers’ self-pay prices for the medication alone, with a refill within 45 days (otherwise Zepbound’s $449 from 7.5 mg is $499 to $699), or $50 a month for eligible Medicare Part D enrollees through the GLP-1 Bridge.

What does “all-in monthly price” mean?

The medication plus every required recurring fee, such as a membership, program or visit fee, and shipping if it’s charged. A program that charges $90 for medication plus a $79 membership is listed at $169. A first-month offer is shown beside the price, not in it, so you compare like with like.

Why do compounded GLP-1 prices vary so much?

Some programs add a membership fee, some raise the price as the dose goes up, and some discount heavily for prepaying several months. Sale prices and first-month offers also move often. That’s why the index shows the month-to-month price, the longest-plan price and the dose policy side by side.

How much do Zepbound and Wegovy cost without insurance?

At the manufacturers’ self-pay prices, for the medication alone: Zepbound KwikPen is $299 to $449 a month at LillyDirect, by dose, and $449 (7.5 to 15 mg) needs a refill within 45 days, or it’s $499 to $699. At NovoCare Pharmacy, the Wegovy pen is $349 ($399 at 7.2 mg) and the pill $149 to $299 (checked November 1, 2026).

How often is this index updated?

At least once a month. The prices on this page were checked on November 1, 2026, except where a row says otherwise. Every check is dated and every change is logged under “What changed,” so you can see how old a price is before you rely on it.

Does the index include every GLP-1 program?

No. It covers the 21 online programs whose prices we could read on their own sites; a price shown only after a quiz, an account or checkout counts as not published. Mondae, which we review separately, isn’t in the tables yet. Every program we add or remove is logged under “What changed.”

How we check

How are these GLP-1 prices checked?

We read each program’s own public pages on November 1, 2026 (PreventiveMD and MEDVi on November 2, 2026), with no quiz, account or checkout. Each price is the medication plus every required fee, month to month; first-month offers are shown beside it. The same rules apply to every provider, NexLife included. How we rate more than price, and NexLife’s own plans: how we compare GLP-1 programs and NexLife pricing.

The details
Where we lookOnly each provider’s own public pages. We don’t take a quiz, create an account or start checkout. A price shown only after those steps counts as not published.
First month (introductory)The introductory price for your first month, listed only where the program shows one and never counted in the month-to-month price; “None shown” means its page shows no first-month offer.
Then, month to month (all in)What you pay each month after any first-month offer, with no commitment: the medication plus every required recurring fee, such as a membership, program or visit fee, and shipping if it’s charged.
Membership feeAny membership or program fee a program requires, shown on its own and already counted in the all-in price; “None” means the program says it charges none, and “None listed” means its page shows none.
Longest plan, per monthThe lowest per-month price a program publishes for a prepaid or multi-month plan, with the plan’s length and, where shown, the upfront total. A dash (–) marks PreventiveMD’s prepaid plans and Embody’s prepaid bundles, which this index doesn’t list.
Price by dose“Same at every dose” means the program says its price doesn’t rise with dose, and “Rises with dose” means it charges more for higher doses, with the range where published. Where the price depends on dose or pharmacy, we show the lowest published figure and say so.
CheckedThe date we last read that program’s own pricing page.
“Not listed”The program’s own pages don’t state it, so we don’t guess.
SalesA sale price is shown as the provider labels it, with its regular price where published.
Billing periodsSome providers bill every 4 weeks and others monthly. We show the price as billed and say which.
Not includedInsurance, promo codes, referral credits and taxes.
Who checksNexLife, reading each provider’s pages on the check date and applying the same rules to every provider, NexLife included. No provider paid to be listed or for its position.
Not yet in this checkMondae, which we review separately (read November 1, 2026), lists compounded tirzepatide at $126 and semaglutide at $87 a month on a 1-month plan, the same at every dose. It isn’t in these tables yet. Sources: Mondae’s own compounded tirzepatide and compounded semaglutide pages, read November 1, 2026; see also our Mondae review.

Step by step

How does the GLP-1 price index check a price, step by step?

The same rules for every provider, NexLife included.

  1. Read the program’s own pages. Only its public pages, with no quiz, account or checkout. A price shown only after those counts as not published.
  2. Add every required fee. Medication plus any membership, program or visit fee, and shipping if charged, month to month with no commitment.
  3. Set first-month offers aside. They’re shown beside the month-to-month price and never counted in it.
  4. Note the dose policy and plans. Whether the price is the same at every dose or rises with it, and the longest plan’s per-month price.
  5. Date every price. Each row shows the date we last read its pricing page. We update at least once a month, logging every change.

No provider paid to be listed or for its position.

Important safety information

Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. A provider prescribes them only when they offer a significant difference for you compared with an FDA-approved product. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Read the full safety information

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath. Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and a faster heart rate. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.

Tirzepatide can make birth control pills less effective: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after you start and for 4 weeks after each dose increase. Both medicines may harm an unborn baby. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider right away if you’re pregnant, planning to be, or breastfeeding.

This is a summary, not the complete safety information. Read the full Prescribing Information for FDA-approved tirzepatide (Zepbound) and semaglutide (Wegovy), and talk with your provider. You can report side effects to the FDA at 1-800-FDA-1088 or fda.gov/medwatch.
